I got tired of doing 1013s at 9 o'clock at night. So I built this.

Fill out the DOH-1013 on your phone at the property, and it goes straight to your Nassau water district.

The 1013 paperwork is the worst part of the job

Handwriting gets kicked back

Rain, grease, or a bad pen and the district can't read it, so it comes back. Miss a field and it comes back too.

It sits in the truck

The 1013 rides around on your dash for a week. By the time it's in, your customer's looking at a shutoff notice.

The back-and-forth

District kicks it back, emails you, you fix it, you resend. Days go by and the customer's calling to ask what's taking so long.

No way to keep track

Which ones did you send? Which devices are due next month? On paper, things slip.

The whole 1013, from your phone.

Punch in your readings

Tap in your pressures and they check themselves as you go. Nothing gets left blank, so a form doesn't come back kicked for a missing field.

Sign on the phone

You and the customer sign right on the screen. No paper, no scanning, no driving back for a signature.

It goes to the district

The 1013 gets built, sent to the right district, and saved before you pull out of the driveway. You can see where every form stands from your dashboard.

Works everywhere

It runs right in your phone's browser. Nothing to download.

Built so the district takes it the first time

Every field the district checks is checked before you hit send.

The district takes it

The official DOH-1013

It's the exact NYS DOH-1013 the district expects, down to the field. Everything required has to be filled in before it'll send, so it doesn't come back. What is DOH-1013? →

Your license, checked

Checked against the state

Your tester license gets checked against the state's records, so you know before test day if it's expiring or lapsed, not after a form gets rejected.

The questions I get on the phone

You're a tester too. Why would you help your competition?
I make my money testing devices, not filling out forms. I'd rather have twenty guys paying $99 a year than fight over one strip mall. And the more of us filing clean 1013s, the better the whole trade looks to the districts.
Is the district going to see my late filings?
No. The district sees the form you submit. That's it. Potaflow isn't a snitch. It doesn't report your timing, your history, or anything you didn't put on the 1013. What you send is what they see.
What if it gets rejected?
If a district kicks a form back, you'll see their notes, fix it, and resend without starting over. And if you're not sure about your first one, call me at (516) 777-0089 and I'll walk it through with you.
